With a connected computer to the DW4000 modems, open up the Direcway Help window.

The serial number will be listed. Write this number in the form above.

GALILEO CUSTOMERS
On a connected computer, enter in the IP address of the Galileo Gateway Modem/Router.

Enter in the login name and password of the gateway.

Click on LAN and enter write in the Hostname “USSD” number in the form above.

Name and relationship of person completing this form. – This information is used to help Ground Control to clarify an issue in case we need to call you. The person listed is the one who would have information on the Ground Control account.
